Manatee Community Foundation is partnering with the team at First 1,000 Days to share information about this collaborative effort to meet the needs of families in Manatee County.

Knowing about this work will be important for any organization in our community that provides services to families or that touches families. We are delighted to know that so many local nonprofits that serve Manatee County are a part of First 1,000 Days already, and now we want to introduce it to others who may not be familiar with the initiative.

We invite you to join us for a demo of Unite Us, the platform that helps send/receive referrals for medical, mental health and social needs for clients. The system is FREE to use and is the technological backbone for First 1,000 Days. The information for the session is below. Join us as we work to see what can be possible when this initiative is extended further into our community.

Free for Nonprofits: Unite Manatee County Information Session

Unite Florida is a coordinated care network of medical, mental health, and social service providers. The platform is sponsored by First 1,000 Days, to meet the needs of families in Manatee County and the tri-county region. Partner organizations in the network are connected through a shared technology platform, Unite Us, which enables them to send and receive electronic referrals for medical, mental health, and social needs for clients/patients. Most importantly, it is a closed loop system that provides visibility into each client’s care journey, tracking and reporting outcomes for every referral and case in the system.
